Preamble
 The health facilities under State control of State Government exist to provide Health care to every citizen of India within the allocated  resources and available facilities.
Objectives
1 -
 To make available  health care services and related facilities for citizens.
2 -
 To provide appropriate advice, treatment ,referral and support that would help to cure the ailment to the extent medically possible.
3 -
 To redress any grievances in this regard.
Commitments  of the charter
1 -
 To provide access to available facilities without discrimination.
2 -
 To provide emergency curative,promotive and preventive care, if needed on reaching health facilities.
3 -
 To provide adequate number of notice board detailing the location of all the facilities and the schedule of fields visit in case of rural  health centers.
4-
 To provide written information on diagnosis, treatment being administered.
5-
 To record complaints and respond at an appointed time.
Grievance redressal
1 -
 Grievances that citizens have will be recorded
2 -
 Aggrieved user after his/ her complaint recorded would be allowedto seek a second opinion at higher unit referral.
Responsibility of the users
1 -
 Users of the health facility  would attempt to understand the commitments made in the charter
2 -
 User would not insist on services above the standard set in the charter because it could negatively affect the provision of the  minimum acceptable level of services to another user.

 Instruction of the health facility personnel would be followed sincerely,and

 In case of grievances, the redressal mechanism machinery would be addressed by users without delay.
 Performance audit and review of the charter
 Performance audit shall  be conducted through a peer review every month by advisory committee of Rogi kalyan samiti of the Health  Facilities. after covering the areasof services  the standards have been specified.
